Not sure what else to try right now. Does it do the same thing on another Mac? Also try, if you haven't, in a fresh account and see if the problem is the same.

i tried it on another mac ... it is the same thing ... i will try to format it ... should i do that in Desk Utilities too?

Yes, just make sure you make a copy of it first.
